Poor planning or execution of the task. <br /> 4. Daydreaming. <br /> Most accidents, outside of an act of God, can be avoided by avoiding the <br /> ' above attitudes naturally, but also by expecting the unexpected. In our <br /> industry of underground construction you must always bare on thing in mind, <br /> you are dealing with Mother Nature. <br /> ' A. Use Care, caution and good sense when performing all your work. Handle, <br /> transport and store things properly. <br /> B. Keep the work area as neat and orderly as is possible. <br /> ' C. Let your supervisor and fellow workers be aware of a potential or <br /> existing hazard. Correct the problem as feasible as is possible. <br /> ' D. Do not handle tools or materials in which you are not familiar with <br /> until you receive proper instruction from an authorized or experienced <br /> employee. <br /> ' E. Horseplay causes accidents, HORSE AROUND LOSE YOUR JOB! <br /> F. Don't stand or walk around moving equipment. <br /> G. BE ALERT AND AWAKE! <br /> 12.03 REPORTING ACCIDENTS <br /> ' In the event you or a fellow employee has an accident, report it to your <br /> supervisor or the office immediately. No matter how small the accident may <br /> seem, PLEASE REPORT IT. Failure to do so may result in your claim being <br /> denied. <br /> ' 12.04 LIQUOR/DRUGS <br /> No liquor, drugs or any intoxicating materials will be allowed at any time on <br /> ' any job, company vehicle or property belonging to Banks & Co. . Anyone <br /> reporting to work in a seemingly intoxicated manner will not be allowed on the <br /> job and may be subject to dismissal. <br /> t12.05 PROTECTIVE EQUIPMENT AND CLOTHING <br /> A. You will be furnished with a hard hat and you are required as a con- <br /> dition of employment to wear it on the job site when needed. <br /> ' B. Personal Protective Equipment (PPE) such as gloves, goggles dust <br /> respirators, rubber boots, ear plugs shall be furnished to any employee <br /> working under conditions that warrant their use. <br /> ' Use of gloves and goggles when chipping, sawing or cutting any material <br /> which may create a hazard to the eyes of hands is required. <br /> C. A safe working shoe shall be worn at all times. Recommended shoes <br /> ' should be a steel toed boot. <br /> D. When working in the vicinity of traffic fluorescent vests will be <br /> provided and you are required to wear them. <br /> ' 11 SITE SAFETY PLAN <br />
