The monitoring plan is not current and/or not approved by the EHD. For the monitoring plan of both underground storage tanks, beneath "Personnel Responsibilities;'the name of the first person having responsibility and their title must be changed. The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a minor violation. 106 CCR 2632(d)(2) Failed to have an approved UST response plan. The response plan is not current and/or not approved by the EHD. In the response plan both names under"Section IV Responsible Persons" need to be removed and updated. Also, in the response plan, "Section VII Owner/Operator signature"needs to be updated accordingly. The response plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, upload the completed form, and submit for review by the EHD. Ensure a copy is available for inspection at any time. This is a minor violation. 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. The site map did not label the 87-and 91-product underground storage tanks. The site map did not indicate the location of the annular sensor and the location of the line leak detectors. The site map also did not indicate float and chains at all 4 dispensers. An accurate site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System(CERS) at http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. This is a repeat violation, Class II. 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. The facility failed to document and address the T2: super unleaded probe out alarm on 10/11/16. Ensure the facility documents and addresses all daily alarms. This is a minor violation. 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator(DO)failed to label the four dispensers correctly on the designated operator reports for 6/11/16 and 7111/16. The DO failed to document and address the T2: super unleaded probe out alarm on 10/11/16. The DO monthly inspection report dated 10/20/16 had a panel print out dated 10/21/16. The DO failed to sign the 3/7/17 DO monthly inspection report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II.
