B. Medical <br /> All contractor and subcontractor field personnel assigned to this operation will <br /> have completed a comprehensive medical examination within the last 12 months. <br /> C. Field Supervision <br /> All field activities will be supervised by a Field Supervisor having training and <br /> experience commensurate with the activity. Field Supervisors have the authority <br /> and responsibility to correct any unsafe act or condition during the course of <br /> operations. <br /> D. Safe Work Practices <br /> All contractor and subcontract personnel assigned to the operation will follow <br /> safe work practices including good personal hygiene, reporting all injuries, <br /> following instructions, inspecting equipment before use, anticipating hazards, <br /> utilizing the buddy system, etc. <br /> E. Job Safety Review <br /> Field Supervisors will conduct daily pre-operations safety meetings with all <br /> contractors and subcontractor personnel. <br /> VII. SPECIFIC HEALTH AND SAFETY REQUIREMENTS <br /> A. Site Control <br /> Work zones and other site control measures will be established as needed for the <br /> operation, depending on chemical hazards, work location and other requirements. <br /> B. Decontamination <br /> When personal protective clothing and equipment (i.e., respirators) are required <br /> for use, standard decontamination procedures shall be followed. <br /> 1. Recommended Procedure <br /> (a) Move to designated area(decontamination station). <br /> (b) Wash boots, gloves and suit, as needed. <br /> (c) Remove tape. <br /> (d) Remove disposable suit (from the inside, out) <br /> (e) Remove boots. <br /> 3 <br />
