i <br /> 10.0 MEDICAL SURVEILLANCE/TRAINING REQUIREMENTS <br /> All personnel must meet the training and medical surveillance requirements specified in <br /> Title 8 CCR Sect. 5192(e)(f). All on-site personnel must have completed the specified <br /> 40 hours of health and safety training. PSI personnel and subcontractors entering the <br /> exclusion zone must be trained in the potential hazards therein, and in safe work <br /> procedures. Copies of training certificates shall be available for review upon request. <br /> 11 .0 EMERGENCY RESPONSE/ACCIDENT INVESTIGATION PROCEDURES <br /> The telephone numbers of the police and fire departments, ambulance service, local <br /> hospital, and PSI representatives are provided in the emergency reference sheet in <br /> Section 1.0. Directions to the hospital are also provided on this sheet and a copy of the <br /> hospital route is provided in Figure 1 . A copy of this information must be posted in <br /> a visible location at the project location before operations commence. The investigation shall be initiated as soon as emergency <br /> conditions are under control. The purpose of this investigation is not to attribute blame, <br /> but rather to assess the pertinent facts so that repeat or similar occurrences can be <br /> avoided. PSI shall assure that accidents/injuries are reported to the appropriate local, <br /> and State authorities. <br /> 12 <br />
