Main St., Manteca, CA 95337 SR0076222 <br /> A request to extend the permit approval to install an underground storage tank (UST) system at the above noted <br /> address was received by the San Joaquin County Environmental Health Department (EHD) on December 19, <br /> 2017. The EHD has extended the permit first issued on December 19, 2017 and which expired on December 31, <br /> 2017. The new expiration date of the permit is December 31, 2018. The (EHD) has completed the review of the <br /> application for a permit to install a new underground storage tank (UST) system at the above noted address. The <br /> proposed underground storage tank (UST) system will consist of one 20,000 gallon 87-octane gasoline UST and <br /> one 20,000 gallon split UST(split between 8,000 gallons of 91-octane gasoline and 12,000 gallons of diesel). <br /> The permit has been approved with the following conditions: <br /> 1. Contact the Lathrop-Manteca Fire District, the San Joaquin Valley Air Pollution Control District, and the <br /> San Joaquin County Community Development Department for applicable permits. <br /> 2. Any changes shall be submitted to the EHD for approval prior to commencing work. All new submittals <br /> supersede the previous ones <br /> 3. Qualified installers must possess or work under the direct and personal supervision of an individual <br /> physically present at the work site, at all times while work is being performed, who possesses all of the <br /> following: <br /> • One of several permissible valid, current contractor's licenses issued by the Contractors State <br /> License Board (may be issued to the installation company), and <br /> • A valid, current UST Installation/Retrofitting certificate issued by the International Code Council <br /> (ICC), and <br /> • A valid, current certificate of training from the manufacturer(s) of the component(s) being installed, <br /> additionally, <br /> • Each person installing or testing any component must hold a certificate of training from the <br /> manufacturer of that component. <br /> 4. Qualified service technicians must possess or work under the direct and personal supervision of an <br /> individual physically present at the work site, at all times while work is being performed, who possesses <br /> all of the following: <br /> • One of several permissible valid, current contractor licenses issued by the Contractors State <br /> License Board (may be issued to the installation company), and <br /> • A valid, current California UST Service Technician certificate issued by the International Code <br /> Council (ICC), and <br /> 1868 E.
