4. All components installed must be certified by an independent testing <br /> organization and bear the appropriate mark as required by the <br /> independent testing organization. <br /> 5. All monitoring equipment must be approved and listed in the State Water <br /> Resources Control Board's Local Guidance Letter 113, "List of Leak <br /> Detection Equipment and methods for Underground Storage Tanks." <br /> 6. Complete and submit a copy of a UST Installation Certificate of Compliance <br /> (Former Form C) for the 87 tank no later than 30 days after work is <br /> completed. <br /> 7. This permit expires 90 days (January 5, 2008) from the approval date <br /> 8. Any changes in information and scope of work must be submitted to EHD for <br /> approval prior to commencing work.
