All LISTS shall be equipped with a spill container <br /> that has a minimum capacity of five gallons and a drain valve that allows drainage of the collected spill into the <br /> primary container or provides a means to keep the container empty. Immediately contact a properly licensed, <br /> trained, and certified contractor to install an approved spill container. A permit is not required from the EHD if the <br /> spill container is replaced, like-for-like. Notify the EHD of the time, date and scope of work by email to <br /> UST@sjcehd.com or fax to (209)468-3433. The service company installing the part must test the system to verify <br /> functionality. The service company must also provide the following documentation to the tank owner/operator for <br /> inspector review during the annual UST inspection: 1. Description of part(s) installed. 2. Name and UST ICC <br /> number, along with manufacturer's certification, of the service technician performing the work. 3. Test results to <br /> verify functionality of the repair. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility operator on the day of inspection. The Environmental Health <br /> Department (EHD) inspector has written the complete report which replaces the initial checklist. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, within 30 days of receiving the complete inspection report. <br /> Note: Communication in under dispenser containment (UDC) 5-6 was verified by opening a port located near <br /> the brine reservoir. Communication must be tested away from the brine reservoir. Immediately contact a <br /> properly licensed, trained, and certified contractor to install the necessary components to test communication at <br /> a location far away from the brine reservoir in UDC 5-6. <br /> Note: A boot in the 87-product submersible turbine pump sump and a boot in the 91-product fill sump were <br /> broken. Immediately contact a properly licensed, trained, and certified contractor to repair/replace the broken <br /> boots under permit and inspection by the EHD. <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br /> Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if <br /> applicable), and the Owner Statement of Designated Operator Compliance.
