The 87-product spill container failed the one-hour hydrostatic test. All LISTS shall be equipped with a spill container that has a minimum capacity of five gallons and a drain valve that allows drainage of the collected spill into the primary container or provides a means to keep the container empty. Immediately contact a properly licensed, trained, and certified contractor to install an approved spill container. A permit is not required from the EHD if the spill container is replaced, like-for-like. Notify the EHD of the time, date and scope of work by email to UST@sjcehd.com or fax to (209)468-3433. The service company installing the part must test the system to verify functionality. The service company must also provide the following documentation to the tank owner/operator for inspector review during the annual UST inspection: 1. Description of part(s) installed. 2. Name and UST ICC number, along with manufacturer's certification, of the service technician performing the work. 3. Test results to verify functionality of the repair. This is a repeat violation, Class II. Overall Inspection Comments: An inspection checklist was provided to the facility operator on the day of inspection. The Environmental Health Department (EHD) inspector has written the complete report which replaces the initial checklist. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, within 30 days of receiving the complete inspection report. Note: Communication in under dispenser containment (UDC) 5-6 was verified by opening a port located near the brine reservoir. Communication must be tested away from the brine reservoir. Immediately contact a properly licensed, trained, and certified contractor to install the necessary components to test communication at a location far away from the brine reservoir in UDC 5-6. Note: A boot in the 87-product submersible turbine pump sump and a boot in the 91-product fill sump were broken. Immediately contact a properly licensed, trained, and certified contractor to repair/replace the broken boots under permit and inspection by the EHD. Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if applicable), and the Owner Statement of Designated Operator Compliance. The UST Owner/Operator: Written Agreement, if applicable, can be stored at the facility or uploaded into CERS.
