(Stantec) is pleased to submit the following proposal to complete the change <br />of ownership permitting notifications for the facilities on the attached list. Permitting to each agency will be as <br />describe below. <br />Introduction <br />RADC Enterprises, Inc. is in the process of acquiring 11 gasoline stations and will need to update the <br />applicable permits for each location upon the close of escrow. A separate application or agency notification <br />is required for each permit as applicable to each facility. <br />Scope of Work <br />Our proposed scope of work includes completion of the permit applications and/or agency notifications for <br />the following agencies, as applicable to each facility. <br />I. CUPA program agency notifications — hazardous materials & UST permits <br />H. Air Quality Management Districts: Air Permits <br />Ill. Department of Weights & Measures <br />A description of each of these permitting requirements is described below: <br />I. CUPA Program Agency Notifications <br />Stantec will completed the CUPA notification by completing the CERS online reporting system. This <br />includes: <br />1. Request access to the existing records by submitting a change of owner notification in CERS. <br />2. Complete all the required fields for the facility, inventory, site maps, emergency plans and the <br />underground storage tanks. <br />3. Stantec will prepare a new site map for each location based on the current aerial images. The <br />new site maps will be created using AutoCAD. <br />
