Stantec will create a new Consolidated Contingency Plan using the latest CERS template for each location. 5. The client shall provide current copies of the following forms: a. Copies of the current UST information and monitoring plans for each tank. b. UST Designated Operator form c. UST Statement of Understanding form d. UST financial responsibility form e. Chief Financial Officer letter, if applicable. 6. Upon completion of all the required data entry fields, Stantec will complete the online submissions for each location. 7. Stantec will forward an electronic Adobe PDF copy of the completed submission to the owner for their records. 8. In the event the regulatory agency denies the submission, Stantec shall coordinate with the owner and the agency to determine the reason for denial and determine a plan of action to remedy the issue. Errors and omission made by Stantec shall be resolved at no additional cost. Depending on the issue, additional costs may apply. Air Quality Management District Permit updates 1. Stantec will contact each air district to obtain the required forms for the change of owner. Some districts have these forms online and are readily available. 2. Stantec will contact the air district to determine the required fees due with each application. 3. Stantec will complete each application required for the change of owner and provide to RADC to submit with the required application fee. Department of Weights and Measures Stantec will contact the regional Weights & Measures agency to confirm the requirements for the owner change. Based on the findings for each facility, Stantec will complete the applicable forms and submit to the agency. If the agency does not require any formal notifications, Stantec will make note of the facilities affected and provide that information to RADC. In some cases, the agency may update their records during the annual inspection of the equipment. CLARIFICATIONS AND ASSUMPTIONS. The following clarifications are provided for the above scope of services: • On-site inspections will not be required to complete the notifications. • Payment of fees is not included in the below costs. Stantec shall notify RADC of any fees required for submission. Stantec can pay the required fees upon request. Standard reimbursement rates shall apply to all fees paid by Stantec. • RADC shall provide the following information for each station: o Tax ID EIN number o UST Board of Equalization numbers o UST Financial Responsibility form o Written notification for the owner change o Date of sale (close of escrow)
