Stantec will create a new Consolidated Contingency Plan using the latest CERS template for <br />each location. <br />5. The client shall provide current copies of the following forms: <br />a. Copies of the current UST information and monitoring plans for each tank. <br />b. UST Designated Operator form <br />c. UST Statement of Understanding form <br />d. UST financial responsibility form <br />e. Chief Financial Officer letter, if applicable. <br />6. Upon completion of all the required data entry fields, Stantec will complete the online <br />submissions for each location. <br />7. Stantec will forward an electronic Adobe PDF copy of the completed submission to the owner for <br />their records. <br />8. In the event the regulatory agency denies the submission, Stantec shall coordinate with the <br />owner and the agency to determine the reason for denial and determine a plan of action to <br />remedy the issue. Errors and omission made by Stantec shall be resolved at no additional cost. <br />Depending on the issue, additional costs may apply. <br />Air Quality Management District Permit updates <br />1. Stantec will contact each air district to obtain the required forms for the change of owner. Some <br />districts have these forms online and are readily available. <br />2. Stantec will contact the air district to determine the required fees due with each application. <br />3. Stantec will complete each application required for the change of owner and provide to RADC to <br />submit with the required application fee. <br />Department of Weights and Measures <br />Stantec will contact the regional Weights & Measures agency to confirm the requirements for the owner <br />change. Based on the findings for each facility, Stantec will complete the applicable forms and submit to <br />the agency. If the agency does not require any formal notifications, Stantec will make note of the <br />facilities affected and provide that information to RADC. In some cases, the agency may update their <br />records during the annual inspection of the equipment. <br />CLARIFICATIONS AND ASSUMPTIONS. <br />The following clarifications are provided for the above scope of services: <br />• On-site inspections will not be required to complete the notifications. <br />• Payment of fees is not included in the below costs. Stantec shall notify RADC of any fees <br />required for submission. Stantec can pay the required fees upon request. Standard <br />reimbursement rates shall apply to all fees paid by Stantec. <br />• RADC shall provide the following information for each station: <br />o Tax ID EIN number <br />o UST Board of Equalization numbers <br />o UST Financial Responsibility form <br />o Written notification for the owner change <br />o Date of sale (close of escrow) <br />
