Critique spill response actions to identify measures to avoid future incidents <br /> and to improve the efficiency of future spill cleanup actions. <br /> 6. Document Response Actions <br /> Reportable oil spills and hazardous materials releases must be carefully <br /> documented so that sufficient information is available to appropriate agencies. <br /> Information concerning the spill should be recorded on the Oil Spill Report and <br /> should include photographs for major spills when appropriate. <br /> Send a copy of the completed Spill Report to the appropriate Environmental <br /> Consultant/Coordinator/Specialist and file the original in the Facility Records. <br /> 7. Follow-up Reporting <br /> Notify the Corporate Law Department of any environmental release (air, ground <br /> or water) that must be reported to a federal, state or local agency; any <br /> environmental release which attracts media attention; or any discovery of <br /> significant historic contamination at a PG&E site. This information must be <br /> reported verbally as soon as possible with written follow up notification no later <br /> than one work day after the incident. <br /> Contact your Environmental Consultant/Coordinator/Specialist for assistance in <br /> ( filing the required written agency notifications described below. <br /> a. Immediately telephone the California Department of Toxic Substances <br /> Control and the local authorities that the cleanup has been completed and <br /> that all emergency response equipment is cleaned, ready for reuse, and <br /> restocked for future use. <br /> b. A written report must be submitted to the Environmental Protection Agency <br /> (EPA) within 60 days whenever a facility has: <br /> i) Discharged more than 1,000 gallons of oil into navigable waters in a <br /> single spill event, or <br /> ii) Discharged oil in two reportable events within any 12.-month period. <br /> The report must be submitted to the Region IX Administrator at the <br /> following address: <br /> Environmental Protection Agency <br /> 75 Hawthorne Street <br /> San Francisco, CA 94105 <br /> l <br /> Page 63 <br />
