The <br /> missing alarms include: L1 91 annular 1/16/16. The alarm was listed on the report, but no response was noted and no <br /> maintenance records were available on site documenting the corrective actions taken in response to this alarm. <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> This is a Class II violation. <br /> 317 HSC 25290.1, 25290.2, 25291 Secondary containment not tight. <br /> The July 14, 2015 secondary containment test results indicate a failure at the 91 annular, UDC 5/6, and UDC 9/10. All <br /> secondary containment for the UST system must be tight. Immediately have a properly licensed, trained, and certified <br /> contractor repair or replace the failed component under permit and inspection of the EHD. <br /> This is a Class 11 violation. <br /> 321 CCR 2636(c)(1) All releases in secondary piping do not flow to a collection sump. <br /> The test boot on the 87 north piping was not pulled back and the valve was pointed straight up. Any potential release <br /> from the primary containment into the secondary containment in the 87 piping does not flow into a collection sump. <br /> All secondary containment systems shall be sloped so that all releases will flow to a collection sump located at the <br /> low point of the underground piping. The boot was pulled back by the technician at the time of inspection. <br /> This was corrected on site. <br /> This is a Class 11 violation. <br /> 323 CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. <br /> The 87 line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting a <br /> 3-gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, and certified contractor repair or <br /> replace the failed leak detector (LG 1 13-x listed, if applicable) and obtain a permit within one business day from the <br /> EHD. If the failed leak detector can't be replaced immediately, there is a possibility that the 87 UST system may be <br /> red tagged to prevent fuel inputs. <br /> This is a repeat violation, Class 11. <br /> Page 5of6 <br />
