Testing has not been <br /> completed for all components and is 8 months past due. According to the test results from July 14, 2015, the 87 <br /> north piping line and the UDC 11/12 were not tested. Secondary containment testing is required once every 36 <br /> months. Immediately schedule this test and provide 48 hours notification to the EHD. <br /> This is a Class II violation. <br /> 110 CCR 2637(e) Failed to submit secondary containment test report within 30 days. <br /> Secondary containment testing was scheduled to be performed on July 14, 2015 and a test report has not been <br /> submitted to the CERS. A copy of the test report must be submitted within 30 days of the test. The test results were <br /> provided by Mr. Grewal at the time of inspection. <br /> This was corrected on site. <br /> This is a repeat violation, Class 11. <br /> 113 CCR 2638(d) Failed to submit monitoring system certification test report within 30 days. <br /> Annual monitoring system certification, leak detector testing, and spill container testing was performed on March 10, <br /> 2015, additional 87 spill bucket testing was performed on March 26, 2015, and a test report for all testing was <br /> submitted on June 8, 2015 , two months late. A copy of the test report must be submitted within 30 days of the tests. <br /> Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. <br /> This is a repeat violation, Class il. <br /> 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br /> Maintenance and monitoring records for the last three years were not found on site. The 2015 monitoring certification <br /> test results were not found on site. Maintenance records for work done in the last year was not found on site, <br /> including work orders for the secondary containment testing done on July 14, 2015 and the response to an L1 91 <br /> annular alarm on January 16, 2016. These records shall be maintained on site for at least three years. Monitoring <br /> records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance <br /> records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of <br /> all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of <br /> inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for <br /> the last three years on site and submit copies of the missing records to the EHD. <br /> This is a Class II violation. <br /> 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible <br /> on site at all times. immediately locate and retain a copy on site. <br /> This is a minor violation. <br /> Page 4 of 6 <br />
