WRITTEN MONITORING AND RESPONSE PLAN <br /> UNDERGROUND STORAGE TANK MONITORING PROGRAM <br /> This monitoring program must be kept at the UST location at all rimes. "The information on this monitoring <br /> program are condition of the operating permit. The permit holder must notify San Joaquin County Environmental <br /> Health Department,phone(209)468-3420 of any changes to the monitoring procedures unless required to obtain <br /> approval before making the change. Required by CCR 2632(d)and 2641(h). <br /> FACILITY NAME Safeway Store 1769 DATE March 1,2003 <br /> FACILITY ADDRESS 2802 Country Club, Stockton, CA 95204 <br /> 1. Name(s)and Title(s)of the people responsible for performing the monitoring and/or maintenance <br /> Stan Olea/Fuel Operations Manager <br /> Veeder Root/Testing and Compliance Manager <br /> Service Stations Inc. <br /> Tanknology <br /> 2. Describe frequency of performing the monitoring for: <br /> Tanks: Continuous Electronic Monitoring <br /> Piping: Continuous Electronic Monitoring <br /> 3. Describe the locations of where the monitoring will be performed. ie location of the sensors <br /> The monitoring panel is located in the interior of the Kiosk <br /> 4. Describe the following equipment for each underground storage tank <br /> Name and Model of Monitoring Panel— Veeder Root TLS-350 <br /> Name and Model of Line Leak Detectors—Veeder Root 848480-001 <br /> Name and Model of Annular Sensors—Veeder Root 794390-460 <br /> Name and Model of Piping Sensors - Veeder Root 794380-352 <br /> Name and Model of Dispenser Sensors—Veeder Root 794380-322 <br /> Type of overfill prevention—Ball Float <br /> 5. Do the Turbine Sensors and Dispenser Pan Sensors shut down the turbine?Yes <br /> Does the turbine shut down if a sensor is disconnected? Yes <br /> Does the turbine shut down if the monitoring panel loses power? Yes <br /> 6. How will the alarm history and subsequent maintenance be recorded? <br /> Alarms are continuously monitored and recorded by remotely by Veeder Root. Additionally,onsite <br /> personnel complete the alarm panel test logs. Maintenance records are filed on site. <br /> 7. Maintenance of the monitoring equipment must be performed at least once every 12 months. Describe the <br /> preventative maintenance schedule and recording for the monitoring equipment. What month will the annual <br /> monitoring certification be performed in? <br /> The monitoring system shall be certified annually in accordance with the manufacturer's recommendations. <br /> System set-up must be checked to verify that the 99% operating mode option has been selected. The annual <br /> monitoring certification is performed in February. <br /> Note: For new installations the secondary containment must be tested 6 months after installation. All secondary <br /> containment systems must be tested once every 36 months with initial testing before December 31, 2003. <br /> 8. Describe the training necessary for the operation of the UST system, including piping, and the monitoring <br /> equipment. <br /> The UST system is monitored 24 hours 7 days per week by Veeder Root. Operators are trained in alarm <br /> response protocol. <br />
