Reporting Format (Check all which apply) ® This facility will use the Unidocs "Hazardous Materials Monitoring System Inspection Form" to document inspections (performed at least monthly) of electronic monitoring systems to ensure that they are in proper working order. ❑ We will use our own documents to record inspections (performed at least monthly) of electronic monitoring systems. (Note:A sample copy of each document used must be attached to this monitoring plan) ❑ A sample copy of each additional UST monitoring form/log used (e.g. alarm logs, MIR forms, etc.) is attached to this monitoring plan. ❑ No in-tank leak detection* methods are employed on these tanks as part of required monitoring; or ❑ Attached to this monitoring plan is a copy of the system set-up report generated by the in-tank leak detection system. * (Note: Inventory management is often performed by the same equipment that performs in-tank leak detection. If you have double-wall tanks, you are not required to include automatic tank gauging as part of yourformal written monitoringprogram) J. Training Copies of the following reference documents are maintained at the facility: (Check any which apply) ® This Underground Storage Tank Monitoring Plan. ® Operating manuals for electronic monitoring equipment. ❑ California Underground Storage Tank Regulations ❑ State Water Resources Control Board (SWRCB) publication: "Handbook for Tank Owners - Manual and Statistical Inventory Reconciliation". ❑ SWRCB publication: "Weekly Manual Tank Gauging for Small Underground Storage Tanks." ❑ Other (specify): Facility personnel with UST monitoring responsibilities have been familiarized with all of the above documents relevant to their job duties. These persons have access to those documents when needed. Personnel responsible for performing inspections are trained to follow a consistent inspection routine and to document inspections in an thorough manner. They are trained to distinguish proper equipment function from obvious hardware or software failures. These personnel have been trained in proper procedures for reporting alarms, leaks, equipment failure, or other problems. Personnel responsible for arranging preventive maintenance, servicing, or calibration of monitoring equipment have sufficient authority to contract such work as may be needed. Personnel responsible for maintaining UST monitoring records (i.e. logs, test reports, equipment service reports, etc.) have been instructed that such records must be maintained on-site, available for inspection, for a minimum of 3 years unless the local agency has indicated otherwise.
