Report completed on December 1, 2011. Checklist left <br /> on site at time of inspection. Violation#302 was added. This report supersedes any previous report. <br /> Class II violations: (204, 311, 313) <br /> 204. On the May 2011 and June 2011 designated operator monthly inspection reports, the designated <br /> operator marked that the secondary containment testing was conducted within the required timeframe. <br /> The secondary containment testing was overdue at the time of both inspections. During the monthly <br /> inspection, the designated operator shall check that all required testing and maintenance for the UST <br /> system have been completed and document the dates they were done. Ensure that designated <br /> operators performing monthly inspections at this facility are including all of the required and correct <br /> information on the reports. <br /> REPEAT VIOLATION <br /> 311. The 87 electronic line leak detector and 91 mechanical line leak failed to detect a leak when tested. <br /> All line leak detectors shall be capable of detecting a 3-gallon per hour leak at 10 psi. The service <br /> technician adjusted the 91 leak detector, retested it, and it passed. Immediately have a properly <br /> licensed, trained, and certified contractor repair or replace the failed 87 leak detector(LG 113-x listed, if <br /> applicable) and obtain a permit within one business day from the EHD. If the failed leak detector can't <br /> be replaced immediately, there is a possibility that the 87 UST system may be red tagged to prevent fuel <br /> inputs. <br /> REPEAT VIOLATION <br /> 313. The 87 and 91 spill containers were observed partially filled with liquid, and failed to have a <br /> minimum capacity of five gallons. All spill containers shall have a minimum capacity of five gallons. <br /> Immediately take all necessary actions to ensure that these containers are capable of a minimum five <br /> gallon capacity. <br /> REPEAT VIOLATION <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> InapeQor.
