Hwy 99, Stockton Program: UST <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Notice to Comply - Minor violations: (108, 117, 302, 304, 306, 307) <br /> 108. Secondary containment testing was last performed on April 10, 2008 and was completed on June <br /> 9, 2011, two months past due. According to the test results submitted for the June 9, 2011 testing, <br /> UDCs 5/6 and 17/18 did not pass. Secondary containment testing was observed for UDC 17/18 on <br /> October 24, 2011. No passing secondary containment test results have been received for either UDC. <br /> Secondary containment testing is required once every 36 months. A copy of the test report must be <br /> submitted within 30 days of the test. Immediately schedule all necessary testing and provide 48 hours <br /> notification to the EHD. Immediately submit a copy of completed test reports to the EHD. Take all <br /> necessary precautions to ensure testing is performed in a timely manner. <br /> 117. According to the maintenance records on site, the 87 CPT was repaired on June 2, 2011 and a <br /> permit was not obtained from the EHD. The CPT for the 87 grade tank also acts as the leak detector. <br /> Emergency repairs relating to UST system monitoring and release detection may be performed by a <br /> properly licensed, trained, and certified contractor pursuant to the EHD's policy and a permit obtained <br /> within one business day from the EHD. All other repairs must have a permit submitted to and approved <br /> by the EHD prior to beginning work. Immediately obtain a permit from the EHD for the 87 CPT. A <br /> "Failure to Obtain a Permit" penalty applies. <br /> 302 & 304. The 91 annular sensor failed to activate an audible and visual alarm when tested and, <br /> therefore, the tank is not monitored. The interstitial space shall be monitored with a monitoring system <br /> connected to an audible and visual alarm system. The sensor was rewired and retested during the <br /> inspection. Obtain a permit from the EHD for this work within one business day. If a permit is not <br /> obtained within one business day, a "Failure to Obtain a Permit" penalty will apply. <br /> 306. A fuel leak was found at dispensers 9/10 and 13/14. Staining was observed and noted in these <br /> same UDCs during the 2008 inspection. All primary containment must be product tight. Immediately <br /> discontinue use of failed piping and have a properly licensed, trained, and certified contractor replace or <br /> repair the failed piping under permit and inspection of the EHD. If the failed component can't be <br /> replaced immediately, there is a possibility that the UST system may be red tagged to prevent fuel <br /> inputs. <br /> 307. The June 9, 2011 secondary containment test results indicate a failure at UDC 5/6. All secondary <br /> containment for the UST system must be tight.
