The monitoring plan is not current and not approved by the Environmental Health Department(EHD).In the UST Tank Information Monitoring Plan for both Tank ID#1 and Tank ID#2, beneath Under Dispenser Containment(UDC) Monitoring, change the UDC Leak Sensor Model from 794380-208 to 794380-352; and beneath Record keeping, change"no"to"yes"for Visual inspection records. For Tank ID#2 (premium unleaded), beneath Pipe Monitoring is Performed Using the Following Methods, change the Leak Sensor Model from 794380-352 to 794380-208. Immediately log into California Environmental Reporting System (CERS) and submit the revised monitoring plan and maintain a copy of the revised monitoring plan on site. This is a repeat violation, Class II. 109 CCR 2637 Failed to test secondary containment. Secondary containment testing was last performed on September 11, 2012 and was performed on September 9, 2015. However, the secondary containment testing of the annular space was not tested according to the manufacturer's specifications until December 1, 2015, three months past due. Secondary containment testing is required once every 36 months. Take all necessary precautions to ensure testing is performed in a timely manner. This is a Class II violation.
