U
D. Results of Testing/Servicing
Software Version Installed: 329687-OOIA/329688-002
Complete the following checklist:
aJ
® Yes
❑No
Is the audible alarm operational?
® Yes
❑No
Is the visual alarm operational?
® Yes
❑No
Were all sensors visually inspected, functionally tested, and confirmed operational?
N Yes
❑No
Were all sensors installed at lowest point of secondary containment and positioned so that other equipment will
not interfere with their proper operation?
7 Yes
❑No
If alarms are relayed to a remote monitoring station, is all communications equipment (e.g. modem) operational?
NN/A
❑ Yes
❑No
For pressurized piping systems, does the turbine automatically shut down if the piping secondary containment
NN/A
monitoring system detects a leak, fails to operate, or is electrically disconnected? If yes: which sensors initiate
positive shut down? (Check all that apply) ❑ Sump/Trench Sensors; ❑ Dispenser Containment Sensors.
Did Von confirm positive shut down due to leaks and sensor failure/disconnection? ❑ Yes; ❑ No.
❑ Yes
[]No
For tank systems that utilize the monitoring system as the primary tank overfill warning device (i.e. no
NN/A
mechanical overfill prevention valve is installed), is the over -fill warning alarm visible and audible at the tank
fillpoint(s) and operating ro erl ? If yes, at what percent of tank capacity does the alarm trigger? %
❑Yes
NNo
Was any monitoring equipment replaced? If yes, identify specific sensors, probes, or other equipment replaced
and list the manufacturer name and model for all replacement parts in Section E, below.
❑Yes
NNo
Was liquid found inside any secondary containment systems designed as dry systems? (Check all that apply) ❑
Product; ❑ Water. If yes, describe causes in Section E, below.
N Yes
❑ No
Was monitoring system set-up reviewed to ensure proper settings? Attach set up reports, if applicable
❑ Yes
N No*
Is all monitoring equipment operational per manufacturer's specifications?
In Section E below, describe how and when these deficiencies were or will be corrected.
E. Comments:
1 -THE DIESEL IS PRESSURIZED BUT HAS NO LLD OR PSD.
2 -THE SOAP TANK HAS NO TLM AND THE ANNULAR SENSOR IS NOT PULLED AROUND THE TANK. THE INSPECTOR
HAS A PERMIT FOR REPAIRS AND IS AWARE OF THE PROBLEM.
3 -TANKS #2 AND #4 HAVE NO PULL STRINGS ON THE SENSOR'S THERE IN PLACE BY A WIRE.
4- NO ALARM HISTORY PAST DEC.27-04 ON UNIT #1, UNIT HAD TO BE REPROGRAMED BY CONTRACTOR DOING
WORK AT SITE.
5 -SECONDARY BOOT ON THE WASTE FUEL TANK # 5 CAN NOT BE PULLED BACK, ITS GLUED IN PLACE.
6- ANNULAR SENSOR ON TK -2 WAS SITTING ON TOP OF THE TANK, AND THE WIRE USED TO FEED THE
SENSOR DOWN WAS GONE THAT WAS INSTALLED IN MARCH OF 04.
* ALL SENSORS FUNCTION NORMALY BUT ANNULAR SENSOR ON SOAP TANK PROHIBITS CERTIFICATION OF
SYSTEM.
