Pescadero Avenue, Tracy — Yellow Transportation, Inc. — SR0037272 <br />The permit application to retrofit UST for SB989 testing dated on September 20, 2004 and <br />February 6, 2004 at the above referenced location has been approved with following conditions: <br />1. Contact the local fire department and San Joaquin Valley Air Pollution Control District for <br />applicable permits and requirements. <br />2. Any changes in information or scope of work must be submitted to EHD prior to <br />commencing work. <br />3. All piping and connections including entry boots shall bear an UL label as provided in the <br />cut sheet. <br />4. All equipment in contact with backfill must have corrosion protection. <br />5. Contact Steven C. Shih at (209) 468-9850 to arrange inspection appointments; 48-hour <br />notice for inspection appointments is required. Contact EHD for the following inspections: <br />(a) Primary piping: pressure and soap test. <br />(b) Secondary piping: pressure and soap test. <br />(c) Piping slope: all piping must slope to a containment sump with an audible and visual <br />alarm. <br />(d) 24-hour lake test on sumps. <br />l (e) Check valve inspection: show the location of check valve to determine the types of <br />suction system. <br />(f) Final inspection. A final inspection must be performed prior to returning this station to <br />C" operation. <br />(i) A third party certified piping line tightness test must be performed immediately <br />after the completion of work and results received by this office within 7 days and <br />prior to the final inspection. <br />(ii) Submit an As -Built plan to EHD at the time of the final inspection if there is any <br />changed in reference to the original approved plan.
