Electrical Conduit <br />In order to permit boot installation, the electrical conduit(s) will cut, inside of the piping <br />sump. Prior to conduit cutting activities, PSI will disconnect the perspective UST <br />monitoring system. Electrical seal offs within sumps will be removed and replaced, as <br />needed. Low -voltage wiring will be pulled back, and restored following boot installation. <br />Conduit will be re -threaded, and fit with Class 1, Division 1, three-piece unions, to <br />permit, and simplify, any future maintenance work. <br />Following the completion of the activities proposed herein, PSI will proceed with the SB - <br />989 testing of each of the UST and piping systems. <br />PSI will schedule each inspection phase directly with the designated SJCEHD <br />representative a minimum of 48 hours prior to the scheduled test date(s). <br />Additional work required beyond this scope of services presented in this work plan will <br />not be performed without prior review and written approval by SJCEHD. <br />Permit Application: <br />PSI has prepared and enclosed the following items for the procurement of UST <br />upgrade and SB -989 testing permits: <br />• SJCEHD Application for Underground Tank Retrofit/Piping Repair Permit <br />• SJCEHD Service Request Application <br />• SJCEHD Underground Storage Tank Program Fee Worksheet <br />• Letter of Authorization, Yellow Roadway Corporation for Professional Service <br />Industries <br />• Site Plan (2) Copies — Subject Property, Locations of Existing USTs <br />• Blueline Technologies 2004 Product Information Catalog <br />• Permit Application Fees Check, for $279.00 USD, Payable to SJCEHD <br />Please call with any questions or concerns you may have, or if PSI can be of additional <br />service, at (562) 597-3977, ext 211.
