~ e 2 <br /> Con[mued <br /> Response: The pipe located in the fill port drop tube of the waste oil UST (T6)that was holding the <br /> overfill prevention flapper open was immediately removed and the vendor who pumps out this UST was <br /> advised not to do this in the future. <br /> Item#23c(1): Designated operator monthly inspection -alarms not documented. <br /> Response: It appears that Tanknology did not document a 12/11/09 U (Tl) fuel alarm or a 12/21/09 L6 <br /> (T2) low liquid alarm in the January monthly DO inspection report. As YRC, Inc.'s designated <br /> operator, I have requested that Tanknology ensure that all alarms are properly documented on the DO <br /> monthly reports in the future. <br /> Item#23d(3): Designated operator employee training—log not kept. <br /> Response: Tanknology did provide training to six personnel on 5/22/09. (See attached photograph of <br /> list of facility personnel who received the DO training by Tanknology on 5/2209 taken from the on-site <br /> compliance notebook.) <br /> Item#27: Annual line tightness test not performed. <br /> Response: All USTs at this facility are either gravity fed or"safe" suction and do not fall under any line <br /> testing requirements, other than SB 989 secondary containment testing requirements. <br /> Please let me know if you need anything else or have any questions. <br /> Sincerely, <br /> uben D.
