Secondary containment testing was performed on 4/27/16 and a test report was submitted on 8/9/16, more than three months late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a minor violation. 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. The facility did not document and address the T-2 piping sump L5 fuel alarm on January 4, 2017 and the T-4 Gear Oil 6000 Probe Out alarm on February 2, 2017. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed at a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Going forth, document all alarms on the daily alarm log, and note what action was taken to address the alarms. This is a minor violation. 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to check and note the test date for secondary containment testing on the 8/31/16, 9/28/16, 11/30/16, and 1/31/17 designated operator monthly inspection reports. There was no date found on the panel printout for the soap panel associated with the 8/31/16 and 12/29/16 designated operator monthly inspection reports. There was no date found on the panel printouts for the soap panel and panel for the remaining six LISTS associated with the 9/28/16, 10/21/16, and 1/31/17 designated operator monthly inspection reports. The L11 alarm history was missing from the panel printout on 1/31/17; and the L7 and L11 alarm histories were missing from the panel printout on 11/30/16. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a minor violation. 210 CCR 2715(e) Designated operator monthly inspection reports not maintained on site for at least 12 months. The July 2016 designated operator monthly inspection reports for both monitoring panels were not found on site; and the January 2017 designated operator monthly inspection report for the soap monitoring panel was not found on site during the inspection. Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies of the previous twelve months of designated operator monthly inspection reports are maintained on site. Submit copies to the Environmental Health Department (EHD)..
