The designated operator employee training for all employees was not current. Unable to located <br /> any Designated Operator training records for 2010 year. Last training log found A'as conducted by <br /> Walton Engineering dated 08/15/09. Elena and Francisco last training documented 08/2009. Mike hired <br /> 2010 but found no documentation of training. Vicky was hired in 1212010; Robert, John, Kyneth- hired <br /> between 0312011 and 0412011 - all received first time training today. Initial training must be conducted <br /> within 30 days of hire. The designated operator shall train facility employees for which he or she is <br /> responsible in the proper operation and maintenance of the UST system once every 12 months. The <br /> training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Emp!oyee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of'.he contact person(s) for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and <br /> submit a copy of the training records to the EHD. <br /> Submit to SJC EHD within 30 days (06/23/11): <br /> 1. a complete copy of the "Return to Compliance Certification" forms <br /> 2. a corrective action statement for the above violations <br /> 3, requested/sLpporting documentations <br /> Notes: <br /> - attached alarm history printouts does not include smart sensors/secondary containment alarms. <br /> Designated Operator has been informed to include these alarms in the monthly reports. <br /> - free training courses offered at SJC EHD. Recommended courses: UST for owners/operators, <br /> Hazardous Waste Management, Universal Waste Management. Schedule found at EHD website. <br /> - onsite: 8:40AM. technician onsite: 8:50AM. offsite: 6:3DPM. received consent for overtime by owner <br /> - today's enforcement compliance monitoring system certification testing will be billed at our current <br /> hourly rate <br /> - Designated Operator Brian Dunahay performed UST employee training for all employees onsite today <br /> - financial responsibility next due by 1112011.
