The Truck Side Diesel South Piping S1 vacuum float switch was repaired on 01/19/11 by Able Maintenarce Work Order 0520756 and a permit was not obtained from the EHD. Diesel South Vent S2 went into alarm 12/17/10 at 1:56PM and it was discovered that the S1 needed to be replaced. Emergency repairs relating t0 UST system monitoring and release detection may be performed by a properly licensed, trained, and certified contractor pursuant to the EHD's policy and a permit obtained within one business day from the EHD. All other repairs must have a permit submitted to and approved by the EHD prior to beginning work. Immediately obtain a permit from the EH for the S1 vacuum float switch. Approximately 2.25 gallons of diesel from North and 1 gallon from South Diesel STP were removed and placed back into the North Diesel Fill sump. Approximately 0 25 inches of liquid was removed from the Truck Side`lent box. Approximately 1 -2 cups of liquid was removed from the Auto Side Vent box. Approximal;ely 0.5 gallons of liquid was removed from the 91 Fill Sump. On 05/22/11, approximately 8 - 9 gallons diesel/water was removed by a facility employee from Truck Side Dispenser 13/148 and Dispenser 15A116B and placed into two 5 gallon hazardous waste containers. All primary containment most be impervious to the liquid and vapor of the substance contained. Immediately discontinue use of failed piping and have a properly licensed, trained, and certified contractor replace or repair the failed piping under permit and inspection of the EHD. If the failed component can't be replaced immediately, there is a possibility that the UST system may be red tagged to prevent fuel inputs. 22. Approximately 8- 9 gallons of diesel/water was discovered and removed from Truck Side Dispenser 13/14B and Dispenser 15A/1613 on 05/22/11 was not recorded. Approximately 2.25 gallons of diesel was removed today from North and 1 gallon South Diesel STP Any release from the primary containment that does not escape the secondary containment and was cleaned up within eight hours of being detected shall be recorded with the following information, 1. Operator's name and phone number 2. List of types, quantities, and concentrations of hazardous substances released 3. Description of actions taken to control and clean the release 4. Method and location of disposal of the released substances 5. Description of actions taken to repair and prevent future releases 6. Description of method used to reactivate interstitial monitoring after repair or replacement of primary containment Immediately record this release with the information required, and maintain it on site with the facility's monitoring recore's. Submit a copy of the recordable release statement to the EHD. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED OATES WILL BE 81LLE0 AT THE CORIIENT HOURLY RATE(§122).
