The Truck Side Diesel South Piping S1 vacuum float switch was repaired on 01/19/11 by Able <br /> Maintenance Work Order 0520756 and a permit was not obtained from the EHD. Diesel South Vent S2 <br /> went into alarm 12/17/10 at 1:56PM and it was discovered that the S1 needed to be replaced. <br /> Emergency repairs relating to UST system monitoring and release detection may be performed by a <br /> properly licensed, trained, and certified contractor pursuant to the EHD's policy and a permit obtained <br /> within one business day from the EHD. All other repairs must have a permit submitted to and approved <br /> by the EHD prior to beginning work. Immediately obtain a permit from the EHD for the S1 vacuum float <br /> switch. A"Failure to Obtain a Permit" double penalty applies. <br /> 43. Approximately 2.25 gallons of diesel from North and 1 gallon from South Diesel STP were removed <br /> and placed back into the North Diesel Fill sump. Approximately 0.25 inches of liquid was removed from <br /> the Truck Side Vent box. Approximately 1 - 2 cups of liquid was removed from the Auto Side Vent box. <br /> Approximately 0.5 gallons of liquid was removed from the 91 Fill Sump. On 05/22/11, approximately 8 - <br /> 9 gallons diesel/water was removed by a facility employee from Truck Side Dispenser 13/14B and <br /> Dispenser 15A/166 and placed into two 5 gallon hazardous waste containers. All primary containment <br /> must be impervious to the liquid and vapor of the substance contained. Immediately discontinue use of <br /> failed piping and have a properly licensed, trained, and certified contractor replace or repair the failed <br /> piping under permit and inspection of the EHD. If the failed component can't be replaced immediately, <br /> there is a possibility that the UST system may be red tagged to prevent fuel inputs. <br /> 22. Approximately 8 - 9 gallons of diesel/water was discovered and removed from Truck Side Dispenser <br /> 13/14B and Dispenser 15A/16B on 05/22/11 was not recorded. Approximately 2.25 gallons of diesel <br /> was removed today from North and 1 gallon South Diesel STP. Any release from the primary <br /> containment that does not escape the secondary containment and was cleaned up within eight hours of <br /> being detected shall be recorded with the following information: <br /> 1. Operator's name and phone number <br /> 2. List of types, quantities, and concentrations of hazardous substances released <br /> 3. Description of actions taken to control and clean the release <br /> 4. Method and location of disposal of the released substances <br /> 5. Description of actions taken to repair and prevent future releases <br /> 6. Description of method used to reactivate interstitial monitoring after repair or replacement of <br /> primary containment <br /> Immediately record this release with the information required, and maintain it on site with the facility's <br /> monitoring records. Submit a copy of the recordable release statement to the EHD. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTIONAT NY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Insp ctor.
