A permit to operate the UST system has not been issued for the 2009, 2010, and 2011 year. No <br /> person may own or operate an UST unless a permit for its operation has been issued by the local <br /> agency to the owner or operator of the UST system. Immediately obtain a permit to operate a UST <br /> system from the EHD. Contact the EHD accounting department for the permit. An "Operating without a <br /> Permit' penalty applies. <br /> 6. A copy of the UST operating permit was not found on site. A copy of the UST operating permit and all <br /> attachments and conditions shall be retained at the facility. Immediately obtain a copy of the permit and <br /> retain it on site. Contact the EHD accounting department for the permit. <br /> 19. The visual light used as part of the overfill prevention system is not currently functional on the Truck <br /> Side audible/visual UST. The light bulb was replaced and retested during the inspection. Passed <br /> functionality test. <br /> 23. Designated operator failed to document all the alarms for the previous month and check that they <br /> were responded to appropriately on the designated operator monthly inspection report. The missing <br /> alarms not documented on the monthly report include: Auto Side: L9 91 Fill Sump alarm on 02/17/11 at <br /> 2:39PM, Truck Side: L2 Diesel 1 South STP annular on 12/17/10 at 10:12AM, 10:52AM. These two <br /> alarms also lacked a response action. Able Maintenance Work Order 0520756 on 12/17/10 documented <br /> S2 no vac alarm. Able Maintenance invoice 508344 on 09/07/10 documented tank 2 sensor vac- no vac. <br /> Service Station System invoice 113066 on 09/25/10 documented tank 2 sensors - no vac. Work Order <br /> and Invoice alarms were not found on the monthly reports. During the monthly inspection, the <br /> designated operator shall review the alarm history for the previous month, check that each alarm was <br /> documented and responded to appropriately, and attach a copy of the alarm history with documentation <br /> taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> 7. Alarm sensor log was not found on site. Any and all alarms must be recorded by employees if they <br /> were not documented by the Designated Operator. No documentation on alarm logs or monthly reports <br /> can be found for the above alarms referenced in violation #23. These records shall be kept on site for at <br /> least 3 years. Immediately locate maintenance records/work orders and maintain the alarm sensor logs <br /> on site. Work orders should be attached to the documented alarms on the Designated Operator's <br /> monthly reports.
