A permit to operate the UST system has not been issued for the 2010 year. No person may own or operate an UST unless a permit for its operation has been issued by the local agency to the owner or operator of the UST system. Immediately obtain a permit to operate a UST system from the EHD. Contact the EHD accounting department for the permit. An "Operating without a Permit' penalty applies. 6. A copy of the UST operating permit was not found on site. A copy of the UST operating permit and all attachments and conditions shall be retained at the facility. Immediately obtain a copy of the permit and retain it on site. Contact the EHD accounting department for the permit. 23a. A new designated operator, Julian Sherrod, had performed the October 2010 monthly inspection report and notification was not provided to the EHD within 30 days of the change. Any changes shall be reported to the EHD with 30 days. Submit notification to the EHD identifying all the designated operators for this facility by 01/08/10. 24. Annual monitoring system certification testing for the truck side was last performed on 10/19/09 and the testing of smart sensors were completed on today, two months past due. These tests are required once every 12 months. Take all necessary precautions to ensure testing is performed in a timely manner. Submit to SJC EHD within 30 days (12/22/10): 1. one copy of the completed 'Return to Compliance Certification" 2. a written corrective action statement for the above violations Notes: - Completing testing of Auto Side - spill buckets and testing of Truck Side smart sensors. - Provided copy of approved monitoring plan and response plan. Ensure copies are kept onsite for review by inspectors upon request. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122).
