All Repeat Violations <br /> 1. A permit to operate the UST system has not been issued for the 2010 year. No person may own or <br /> operate an UST unless a permit for its operation has been issued by the local agency to the owner or <br /> operator of the UST system. Immediately obtain a permit to operate a UST system from the EHD. <br /> Contact the EHD accounting department for the permit. An "Operating without a Permit' penalty applies. <br /> 4. Financial responsibility documents have not been submitted to the EHD for the 2010 year. Last <br /> financial documents submitted to EHD was the Certification of Financial Responsibility (CFR) and Letter <br /> from Chief Financial Officer (CFO)was dated 08/10/09. CFR and Letter from CFO only valid for one <br /> year. Current financial responsibility documents are required to be submitted annually. Complete and <br /> submit a copy of the financial responsibility by 11/29/10. <br /> 5 & 6. Current approved copies of the monitoring and response plans were not found on site. The <br /> monitoring and response plans must be current and approved by the EHD. Complete and submit a copy <br /> of the monitoring and response plans for approval by the EHD by 11/29/10. An approved copy will be <br /> returned for your records. <br /> 6. A copy of the UST operating permit was not found on site. A copy of the UST operating permit and all <br /> attachments and conditions shall be retained at the facility. Immediately obtain a copy of the permit and <br /> retain it on site. Contact the EHD accounting department for the permit. <br /> 7. The Monitoring System Certification, Line leak detector and Spill bucket test results for 2009 and 2008 <br /> were not found on site. These records shall be kept on site for at least 3 years. Contact the testing <br /> contractors to obtain test results. Immediately locate and maintain the two missing above documents on <br /> site by 11/29/10. <br /> 17. The 91 line leak detector failed to detect a leak when tested. All line leak detectors shall be capable <br /> of detecting a 3-gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, and <br /> certified contractor repair or replace the failed leak detector(LG 113-x listed, if applicable) and obtain a <br /> permit within one business day from the EHD. If the failed leak detector can't be replaced immediately, <br /> there is a possibility that the 91 UST system may be red tagged to prevent fuel inputs. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILLBE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY ME EHD'S CURRENT HOURLY RATE. <br /> EHD In or: Received Title: <br /> c <br /> EHD 23-02-003 Rev 10/10 , CONTINUATION FORM <br />
