All Repeat Violations <br /> 23c. Designated operator(DO) failed to document all alarms and check if the alarms were responded to <br /> in the monthly visual inspection report. DO Julian Sherrod did not document the following alarms in his <br /> October 28, 2010 DO report: 1-4: 91 STP Brine fuel alarm on 10/01/10 at 11:52 AM and 9:29 AM; S4: 91 <br /> Prod Line Vacuum Warning on 10/01/10 at 10:30 AM, No Vacuum Alarm on 09/29/10 at 2:25 PM; S5: 91 <br /> Vent Line Vacuum Alarm on 10/01/10 at 9:26 AM, No vacuum alarm on 09/28/10 at 2:31 PM and <br /> 09/21/10 at 4:07 PM. <br /> DO Jessie Wadkins failed to document the S4: 91 Prod Line had No Vacuum Alarm on 02/17/10 at 8:45 <br /> AM on February 28, 2010 DO report. DO Greg Copp stated no work order found for the S5: 91 Vent Line <br /> No Vacuum alarm on 01/05/10 at 10:50 AM. There was no other corrective response action taken for <br /> this alarm. During the monthly inspection, the designated operator shall review the alarm history for the <br /> previous month, check that each alarm was documented and responded to appropriately, and attach a <br /> copy of the alarm history with documentation taken in response to any alarms to the monthly report. <br /> Ensure that designated operators performing monthly inspections at this facility are including all of the <br /> required information on the reports. <br /> 43. Approximately 2 cup of Diesel fuel was found at the bottom of the Diesel STP sump on both sides of <br /> the saddle and removed by technician. Approximately 1 inch of liquid was found in the vent box. All <br /> primary containment must be impervious to the liquid and vapor of the substance contained. <br /> Immediately discontinue use of failed piping and have a properly licensed, trained, and certified <br /> contractor replace or repair the failed piping under permit and inspection of the EHD. If the failed <br /> component can't be replaced immediately, there is a possibility that the diesel UST system may be red <br /> tagged to prevent fuel inputs. <br /> Submit to SJC EHD within 30 days (12/22/10): <br /> 1. one copy of the completed "Return to Compliance Certification" <br /> 2. a written corrective action statement for the above violations <br /> Notes: Designated Operator (DO) Julian Sherrod was not noted on the Owner Statement for DO. Submit <br /> within 30 days of when Julian performed his October 28, 2010 monthly report. <br /> - Siphon Jet Valve on 91 turbine changed out onsite due to high back pressure. <br /> -Technician unable to test 87, 91, Diesel Spill buckets (Auto side) today nor complete MSC testing from <br /> 10/27/10 (Truck side) for smart sensors (S1-S4). Immediately schedule to finish the MSC testing. <br /> Provide 48 hours notification to EHD.
