All Repeat Violations <br /> 22 continue. after detection. A copy Reportable Release Report will but submitted to SJC EHD by <br /> 11/29/10 in lieu of Recordable Release Report if cleanup is not done within 8 hours. <br /> 23c. The Designated Operator failed to document a response action for or check that they were <br /> responded to appropriately on the 02/26/10 designated operator monthly inspection report. DO stated <br /> "L5 Diesel South Fill Sump is currently in alarm" regarding the fuel alarm on 01/21/10. There were no <br /> work orders or response action for the L5 fuel alarm found. During the monthly inspection, the <br /> designated operator shall review the alarm history for the previous month, check that each alarm was <br /> documented and responded to appropriately, and attach a copy of the alarm history with documentation <br /> taken in response to any alarms to the monthly report. Ensure that DO performing monthly inspections <br /> at this facility are including all of the required information on the reports. <br /> 43, 44. All diesel dispensers have weeping at the union above the shear valve piping. All the under <br /> dispenser pans has what appears to be diesel fuel: Disp 18a -wet, small liquid amounts, dark <br /> appearance; Disp 17a - damp moisture; Disp 16a -wet, small liquid amounts; Disp 15a -wet, small <br /> liquid amounts, light appearance; Disp 14a-wet, one inch of liquid, fuel odor. Diesel fuel was found in <br /> the Diesel North and South STP sump. All primary containment must be impervious to the liquid and <br /> vapor of the substance contained. All secondary containment shall be impervious to the liquid and vapor <br /> of the substance contained and constructed to prevent structural weakening as a result of contact with <br /> any hazardous substances released from the primary containment. Immediately have a properly <br /> licensed, trained, and certified contractor find the source of the leak and replace or repair the failed <br /> equipment. A permit and inspection of the EHD maybe required. If the failed component can't be <br /> replaced immediately, there is a possibility that the Diesel North and South UST system may be red <br /> tagged to prevent fuel inputs. <br /> Submit to SJC EHD within 30 days (11/29/10): 1. one copy of the completed "Return to Compliance <br /> Certification" form 2. a written corrective action statement for the above violations 3. requested/ <br /> supporting documentations <br /> Note: A partial annual Monitoring System Certification (MSC) was performed on 10/27/10. 1-1-1-24 <br /> sensors were completed. Technician was unable to complete testing of the smart sensors (S1-S4). A <br /> complete MSC testing due due by end of October 2010. Immediately schedule to finish the MSC testing <br /> and provide 48 hours notification to the EHD. Please be aware that a follow up inspection will be <br /> charged at our current hourly rate, different rate of charge if testing performed on weekend. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WI BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY ME EHD'S CURRENT HOURLY RATE. <br /> EHD or: Recei Title: <br /> J <br /> EH 2-003 08/10110 CONTINUATION FORM <br />
