This information was required to be submitted by November 19, 1009 and <br /> immediately after the December 28, 2009 inspection. This information has not been received <br /> resulting in a non-compliant status for this facility. <br /> Failure to correct the violations will result in continuing/additional violations. <br /> Note: All EHD staff time associated with failing to comply, including the issuance of <br /> this letter, is billed at the current hourly rate ($115).
