All Truck Side diesel dispenser pan, North & South STP sumps and Auto Side diesel STP sump <br /> have what appears to be diesel fuel. Any unauthorized release from primary containment which UST <br /> operator is able to clean up within 8 hours after the release was detected or should be detected, and <br /> which does not escape from secondary containment, does not increase hazard of fire or explosion, and <br /> does not cause any deterioration of secondary containment, must be recorded in facility's monitoring <br /> plan. Report information must include: <br /> 1. Owner's/operator;s name & telephone number, 2. List of types, quantities, and concentration of <br /> hazardous substance released, 3. Description of action taken to control & stop the release, 4. Method & <br /> location of disposal of hazardous substance. Attach copies of any hazardous waste manifest. 5. <br /> Description of proposed methods of repair or replacement of primary or secondary containment, 6. <br /> Description of additional action taken to prevent future releases. A copy of Recordable Release Report <br /> must be submitted to SJC EHD if clean up is within 8 hours after detection. A copy of Reportable <br /> Release Report will be submitted if clean up is not done within 8 hours. A weekly Recordable Release <br /> Report should be documented every Saturday by the designated employee removing diesel from Truck <br /> Side dispensers. It should specify amounts of liquid removed from each dispensers. <br /> 406. The L10: 91 Fill Brine sensor for the double wall brine sump failed to clear out of alarm after being <br /> tested. The sensors monitoring the liquid level in the interstitial space shall be maintained so that a <br /> breach in the primary or secondary containment is detected before the hazardous substance is released <br /> into the environment. Correct immediately by having a properly licensed, trained, and certified <br /> contractor replace the failed component with a functional component (LG 113-x listed, if applicable) and <br /> obtain a permit within one business day from the EHD. If the failed component can't be replaced <br /> immediately, there is a possibility that the 91 UST system may be red tagged to prevent fuel inputs. A <br /> permit is taken out by contractors Service Station System and has been approved on June 27, 2012. No <br /> replacemenUrepairs has yet to be made as of today. L10 alarm is still active on Auto Side monitoring <br /> system. <br /> Submit to SJC EHD within 30 days (08/30/12): <br /> 1. one copy of the completed "Return to Compliance Certification" form <br /> 2. a written corrective action statement for each of the violations <br /> 3. requested /supporting documentations <br /> Notes: Lithium backup battery replaced on Truck Side Veeder-Root monitoring system due to system <br /> failed to return back to power back on after failed safe test. Cold start was performed, only today's alarm <br /> is present. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector.
