The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements. <br /> 2. Individuals installing UST Systems or components shall possess or work under the direct and <br /> personal supervision of an individual physically present at the work site who possesses all of <br /> the following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), and <br /> b. A valid, current International Code Council (ICC)UST Installer/Retrofitting certificate <br /> and <br /> c. A valid, current certificate of training from the manufacturer(s)of the component(s) <br /> being installed. <br /> 3. Qualified service technicians shall possess or work under the direct and personal supervision <br /> of an individual physically present at the work site who possesses all of the following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), and <br /> b. A valid, current California UST Service Technician certificate issued by the <br /> International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s)of the component(s) <br /> being tested. <br /> 4. Each individual installing or testing any component shall hold a certificate of training from the <br /> manufacturer of that component. <br /> 5. Any changes shall be submitted to the Environmental Health Department(EHD)for approval <br /> prior to commence work. All new submittals supercede the previous ones. <br />
