The designated operator documented the L10 brine alarms from the 11/01/17 designated operator monthly inspection report, but failed to check that they were responded to appropriately. The work order by B.Z. Maintenance dated 10/23/17 indicates there is a possible brine leak. On the 5/12/17 designated operator monthly inspection report, brine was added to dispenser 3/4, but no additional follow up was provided. During the monthly inspection, the facility shall attach a copy of the alarm history with documentation taken in response to any alarms and issues to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a minor violation.
