More documentation was <br />provided on November 16, 2016. A second review has been completed. Please provide the following <br />additional underground tank system information. <br />1. The installing technician's certifications. At least one person shall have ICC and Manufacturer <br />certifications on site during installation. Certifications needed: ICC UST Installer / Retrofit, ICC <br />Service Technician, NOV Pipe and Penetration Fitting, Bravo Sump and Penetration Fitting, <br />Veeder Root TLS-350, OPW Spill Bucket, Flex-ing Pipe, Hose Master Pipe, Xerxes Tank. The <br />EHD has not received these certifications although the Nov 15, 2016 Barghausen letter said they <br />were sent. <br />2. Explain how communication can be demonstrated throughout the vacuum, pressure or <br />hydrostatic (VPH) monitored interstitial spaces of the vent line transition sump, under dispenser <br />containment (UDC) sumps, tank top sumps, vacuum-monitored lines. Vacuum or hydrostatic <br />liquid communication must be demonstrated at the far ends of all interstitial spaces. The end of <br />the piping vacuum zone under each UDC must be tested. Each double-walled hydrostatic sump <br />must be tested throughout the sump. Include a drawing of all test ports used to test <br />communication. Typically, a minimum of two test ports in each hydrostatically monitored sump, <br />UDC and vent sump is necessary to verify communication of the interstitial fluid throughout the <br />sump. The provided cut sheets and drawings show only one port for each hydrostatically <br />monitored sump, which is not adequate. <br />3. Submit a drawing of the vacuum monitored piping interstitial spaces and label each zone with the <br />corresponding vacuum sensor. Page G.0.2.0 shows the piping layout but does not show how <br />many vacuum sensors will be installed and which zones each vacuum sensor will be monitoring. <br />Also indicate the location of the test ports at the end of each vacuum zone to test communication <br />throughout the zone. <br />4. The application and plans list NOV Fiberglass Systems pipe. Please add the model / brand. Red <br />Thread IIA? The EHD has not received an updated application although the Nov 15, 2016 <br />Barghausen letter said it was sent. <br />5. To the application please add the manufacturer and brand/model of tank and piping. See the <br />enclosed highlighted application.
