Annual monitoring system certification, leak detector testing, and spill container testing was <br /> performed on May 17, 2012, and a test report was not received within thirty days by the EHD. The test <br /> report was received on July 19, 2012. A copy of the test report must be submitted within 30 days of the <br /> tests. Take the necessary precautions to ensure that a copy of the test report is submitted to the EHD <br /> within thirty days of the test. <br /> 113. A new designated operator was hired on September 11, 2012, and notification was not provided to <br /> the EHD within 30 days of the change. Any changes shall be reported to the EHD within 30 days. Submit <br /> notification to the EHD identifying all the designated operators for this facility by May 10, 2013. <br /> 201. The sensor#2 in the north turbine sump alarmed on December 4, 2012. The designated operator <br /> did not document this alarm on the monthly designated operator report and the facility alarm lag was not <br /> on site. Thomas Penry explained that an inventory print out is placed into the "Weekly Monitoring UST <br /> Binder" and that any alarm print outs would also be placed into this same binder. These records shall be <br /> kept on site for at least 3 years_ Immediately locate and maintain the facility alarm log on site and submit <br /> copies to the EHD by May 10, 2013. <br /> 204. The designated operator failed to document all the alarms from the attached alarm history on the <br /> December 7, 2012, designated operator monthly inspection report and failed to check that they were <br /> responded to appropriately. The missing alarm includes: sensor#2 north turbine sump alarm. During the <br /> monthly inspection, the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history <br /> with documentation taken in response to any alarms to the monthly report.
