Financial responsibility documents which list Verizon Business 2500 W. Turner Rd. Lodi, CA <br /> 95242 have not been submitted to the EHD. Current financial responsibility documents are required to <br /> be submitted annually. Our records indicate that financial responsibility documents which list Verizon <br /> Business 2500 W. Turner Rd. Lodi, Ca 95242 expired 12-1-12. The current copy received does not <br /> include Verizon Business 2500 W. Turner Rd. Lodi, CA 95242 in the attached list of insured facilities. <br /> Immediately submit an updated copy of the financial responsibility documents to the EHD which include <br /> Verizon Business 2500 W. Turner Rd. Lodi, CA 95242. Alternatively, log into CERS, upload the required <br /> information, and submit for review by the EHD. Complete by May 9, 2014. <br /> 108. Secondary containment testing was last performed on 5-14-10 and was completed on 9-10-13, <br /> four months past due. Secondary containment testing is required once every 36 months. Take all <br /> necessary precautions to ensure testing is performed in a timely manner. <br /> 109. The line test which is part of the annual monitoring system certification was scheduled to be <br /> performed on 5-24-13 and a test report has not been submitted to the EHD. A copy of the test report <br /> must be submitted within 30 days of these tests. Immediately submit a copy of the test report or <br /> schedule these tests and provide 48 hours notification to the EHD. <br /> 303. The south turbine sump sensor was located on the west side of the sump and not located to <br /> detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be able to detect a <br /> leak at the earliest possible opportunity. Immediately relocate this sensor to the lowest point of the east <br /> side of the south turbine sump which is where the product piping penetrates the sump wall. Do this so <br /> that if any fuel leaks into the product secondary piping, then the south turbine sump sensor would be <br /> able to detect the leak at the earliest opportunity. If the sensor must be rewired to reach the lowest point <br /> of the east side of the south turbine sump, contact a properly licensed, trained, and certified contractor to <br /> perform the repair under permit and inspection of the EHD. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> Inspector Receiv y: Title) <br /> EHD 23-42 Rev 04/19/12 Page 3 UST INSPECTION REPORT <br />
