The permittee shall maintain on the premises a log of any repairs made to the certified Phase I or Phase 11 vapor <br /> recovery system. The repair log shall include the following: 1)date and time of each repair; 2)the name and <br /> applicable certification numbers of the person(s)who performed the repair, and if applicable,the name, address and <br /> phone number of the person's employer; 3)description of service performed;4)each component that was repaired, <br /> serviced, or removed; 5)each component that was installed as replacement, if applicable; and 6)receipts or other <br /> documents for parts used in the repair and,if applicable,work orders which shall include the name and signature of the <br /> person responsible for performing the repairs. The O&M manual shall be kept at the dispensing operation and made available to any person who operates,inspects, <br /> maintains,repairs,or tests the equipment at the operation as well as to District personnel upon request.
