HAZARD ANALYSES <br /> List all activities in the job activity column and identify how each category of hazard exists at each activity (see <br /> example hazard analyses,Appendix A). <br /> Job Task Mechanical Electrical Chemical Temperature <br /> 1. <br /> 2. <br /> 3. <br /> 4. <br /> 5. <br /> 6. <br /> 7. <br /> 8. <br /> 9. <br /> 10. <br /> HAZARD MITIGATION <br /> Identify procedures to mitigate all hazards listed in the section above by placing the task number next to the <br /> appropriate mitigation measure. A specific procedure must be entered to mitigate each hazard identified in the <br /> previous section. <br /> Activity <br /> List No. A. Mechanical Hazards <br /> Do not stand near backhoe buckets and earthmoving equipment. <br /> Verify that all equipment is in good condition. <br /> ✓ Do not stand or walk under elevated loads or ladders. <br /> Do not stand near unguarded excavations and trenches. <br /> Do not enter excavations or trenches over five (5) feet deep that are not properly secured. <br /> ✓ Check for any other mechanical hazards that might exist. <br /> Check for any underground utilities. <br /> B. Electrical Hazards <br /> Locate and mark buried utilities before drilling. <br /> Utilities located by PG&E on <br /> ✓ Maintain at least a ten foot (loft.) clearance from overhead power lines. <br /> Contract utility company for minimum clearance for high voltage power lines. <br /> If unavoidably close to buried or overhead power lines,have power turned off, with circuit <br /> breaker locked and tagged. <br /> Properly ground all electrical equipment. <br /> �G Avoid standing in water when operating electrical equipment. <br /> Ll� Be familiar with specific operating instructions for each piece of equipment. <br /> C. Chemical Hazards <br /> v Use personal protective equipment indicated on Site Safety Checklist. <br /> Conduct direct reading air monitoring to evaluate respiratory and explosion hazards (list <br /> instrument, action level, monitoring location, and action to be taken). <br /> Consult regulatory agencies for personal air monitoring. <br /> NE=Not Expected <br /> D.
