An"Underground Storage Tank Certification of Installation/Modification" form(form C) <br /> shall be submitted by the owner or the owners'agent [pursuant to(CCR)2635(d)] before <br /> this upgrade will be considered complete by EHD. <br /> 9. All components must be certified by an independent testing organization and labeled as <br /> required by the organization. (UL,ULC,ANSI,API,ASTM,etc. See California Code of <br /> Regulations(CCR),Title 23 Chapter 16 Table B for complete list). The component must <br /> bear the appropriate mark as required by the independent testing organization. <br /> 10. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid,current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company), and <br /> b. All valid, current California UST certificates issued by the International Code <br /> Council(ICC),that are applicable to the work being performed, and <br /> c. A valid,current certificate of training from the manufacturer(s)of the <br /> component(s)being tested,repaired,or serviced; <br /> and <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br /> 11. Permit expires 180 days from date of approval.
