The <br /> permit has been approved with the following conditions. <br /> Conditions of approval: <br /> I. Contact the local fire department, local planning or building department and the San <br /> Joaquin Valley Air Pollution Control District for any applicable permits. <br /> 2. Any changes shall be submitted to the San Joaquin County Environmental Health <br /> Department(EHD)for approval prior to commencing work.All new addenda supercede <br /> the previous ones. <br /> 3. If the alarm history is cleared by a"cold start", a current printout must be created <br /> and available for EHD inspection. Inspection may occur during the monitoring system <br /> functionality test. <br /> 4. If the under dispenser containment(LDDC)pans are being modified to add electrical <br /> penetration fittings and the electrical penetrations are not significantly higher than the <br /> product line penetrations, an approved test must be performed to determine the integrity <br /> of the UDC pans. <br /> 5. All possible piping leaks in the new dispensers must flow into the LIDCs. <br /> 6. Contact Garrett Backus (209468-2986)to arrange the following inspection(s) (a 48-hour <br /> notice is required): <br /> a. The EHD must witness the testing of the functionality of the monitoring system <br /> to ensure that monitoring has been restored to meet the requirements of <br /> California Code of Regulations Title 23 and the Health& Safety Code. <br /> b. The EHD must inspect the dispensers and UDCs. <br /> 7. If concrete is being broken near the UST system,pipeline and tank integrity testing must <br /> be performed by a SWRCB licensed tank tester in accordance with the manufacturer's <br /> guidelines. Results must be received by the EHD within seven days of returning the <br /> facility to operating condition. <br /> Page 1 of 2 <br />
