jL) <br /> HEALTH AND SAFETY MANAGER(HSM) <br /> E: <br /> The HSM has the primary responsibility for ensuring that the Policies and Procedures of <br /> this HASP are implemented. The HSM ensures that all personnel designated to work at <br /> the site are qualified according to Medical Surveillance and Health and Safety Training <br /> consistent with OSHA's 29 CFR 1910.120 Regulations covering Hazardous Waste <br /> Operations and Emergency Response. The HSM is responsible for providing the <br /> appropriate monitoring and safety equipment necessary for the implementation of this <br /> HASP. Any significant changes to the HASP must be approved by the HSM. <br /> SITE SAFETY AND HEALTH OFFICER (SSHO) <br /> The SSHO, Lead TES Employee on-site, has the responsibility to implement this HASP. <br /> The SSHO will investigate all accidents and incidents occurring at the site. The SSHO <br /> will conduct safety briefings and site-specific training for on-site personnel. The SSHO <br /> will accompany all OSHA and other Governmental Agency Personnel visiting the site in <br /> response to health and safety issues. He is responsible for modifying and/or developing <br /> new procedures, after consultation with the HSM, when site or environmental conditions <br /> change due to natural causes or the site operation. <br /> The SSHO has Stop-Work Authorization if they determine that an imminent safety <br /> hazard or potentially dangerous situation exists. Authorization to Proceed with Work <br /> will be verified by the HSM. The SSHO will be responsible for implementing evacuation <br /> procedures, including the shutting down of appropriate equipment, removing equipment <br /> from areas, and coordinating necessary emergency services on-site. <br /> No work is to be performed without the SSHO or HSM on site, unless an alternate SSHO <br /> or HSM has been approved in advance by the Project Manager, Kristin Reynolds, Tait <br /> Environmental Services, Inc. <br /> TEAM MEMBERS <br /> All team members, including all employees of subcontractors working at the site, will be <br /> responsible for understanding and complying with all site Health and Safety <br /> Requirements. All team members will have been provided with any site-specific training <br /> regarding the hazards involved for this particular project. Subcontractors will be <br /> responsible to provide their employees with appropriate equipment and clothing. <br /> Interface of trades involved with construction will occur. Communication between the <br /> trades will exist to point out the site-specific potential physical and/or chemical hazards. <br />
