Tait shall have the appropriate fire extinguisher equipment on the job prior to the start of the project. No cutting torch, open flame, or spark producing equipment shall be used on the tank or piping at any time. Any source of ignition shall be at least 50 feet away from the excavation site. A properly calibrated flammable/combustible gas and oxygen analyzer is required on the job site prior to excavation or removal. Calibration of the meter must be within the last three months. All electrical lines, piping, and like items known to be associated with the tank shall be isolated or disconnected. If at anytime during the Removal of AST, the lower explosive limit of the atmosphere around the tank exceeds 25 percent, all work will cease and proper mitigation procedures initiated. CONFINED SPACE DO NOT ENTER TANK SUMP (S) FOR ANY REASON. Appropriate control measures, such as fencing and confined space supervision, harness &tripod must be employed prior to any worker entering into the tanks sumps. All tank sumps and some excavations are considered permit required confined spaces until they are removed, and after the new sumps are installed and the concrete tank slab has been poured. Monitoring and all other confined space procedures must be followed when entering the sumps. THERE ARE NO EXCEPTIONS SOIL REMOVAL, DISPOSAL AND SAMPLING Any and all soil removal and disposal is the sole responsibility of the client, and all soil samples will be analyzed per clients' requirements. LOCKOUT AND TAGOUT PROCEDURES (ENERGY ISOLATION) Lock-out procedures must be followed during the cleaning, servicing, or adjusting of machinery or electrical equipment. LOTO procedures shall be implemented as required by applicable Federal, State, or Property Owner rules and regulations. FALL PREVENTION 100% fall protection shall be employed for all work at heights. Fall protection shall include scaffolds, aerial lifts, and personnel hoists. Also guardrails, toe boards, and other primary systems may be implemented. In addition, provisions for secondary fall protection (full-body harness and fall arrest systems) shall be used for employees working or traveling more than six (6) feet above a lower level and are exposed to an imminent fall hazard (this includes working conditions hat present hazards such as leading edge, open holes, and skylights on building rooftops and/or eI r elevated towers/structures).
