employee identified in your report to confirm whether they had ever worked at the <br /> Gas Station and, if so, when they ceased working at the Gas Station. All of the <br /> employees listed in your report were employees who worked in other warehouse <br /> departments and a majority of these employees had covered breaks for Gas <br /> Station employees in the past but were no longer covering breaks at the Gas <br /> Station at the time of the inspection. Seth Alexander, Andres Cabrera, Jayson <br /> Thompson, Roy Hampton, Adrian Bobadilla, and Elgie Grant, however, <br /> confirmed that they had been covering breaks at the Gas Station at the time of <br /> the inspection. The Belshire training records indicate that these six employees <br /> have not been trained by a Designated Operator within the last 12 months. The <br /> remaining employees identified in your report had either never covered breaks at <br /> the Gas Station or were no longer covering breaks at the Gas Station at the time <br /> of the inspection. <br /> Following your inspection, Jayson Thompson was recertified by a Designated <br /> Operator on 9/23/11, and Roy Hampton and Adrian Bobadilla were recertified on <br /> 10/4/11. Seth Alexander, Andres Cabrera, and Elgie Grant are no longer <br /> permitted to cover breaks at the Gas Station and are not listed on the posted Gas <br /> Station Employee list. <br /> To ensure that every Gas Station employee receives Designated Operator <br /> training every 12 months, the Gas Station Supervisor will review the posted Gas <br /> Station Employee list monthly to confirm that every employee on the list has <br /> received training from a Designated Operator within the last 12 months in <br /> accordance with Belshire's Designated Operator Training Log. Any employee <br /> whose training has expired will be immediately deleted from the Gas Station <br /> Employee list. The Assistant Manager over Ancillaries will serve as a backup on <br /> this review if the Gas Station Supervisor is not available and the Assistant <br /> Manager over Merchandising will serve as a second backup. Through this <br /> system, the Gas Station will ensure that all of its employees receive Designated <br /> Operator training on an annual basis. <br /> A copy of the completed Return to Compliance Certification signed by Todd <br /> Jerman, the Tracy Warehouse Manager, is included. <br /> #658 Tracy Response 3 of 4 <br />
