11. All installation and testing shall be in compliance with manufacturer's instructions, <br />section 2635 and section 2636 of California Code of Regulations (CCR). <br />12. All tank top openings that are connected to the primary space of the UST must have <br />secondary containment, audible/visual alarm and be vapor tight or terminate at grade. <br />13. Sumps and under dispenser containment must be constructed in a manner to prevent <br />vapors from entering the backfill. <br />14. All secondary containment must be constructed to prevent water intrusion. <br />15. The secondary containment system must be continuously monitored as required under <br />California Health & Safety Code (HSC) 25290.1 <br />16. Overfill prevention shall satisfy CCR 2635(b)(2)(C) pursuant to the permit application. As <br />agreed by applicant ball floats will not be used on vents for overfill prevention. <br />17. Product line leak detector (PLLD) line length measurements shall be provided to confirm <br />the accurate monitoring panel programming. <br />18. All metal components in direct contact with the ground such as flexible connectors, <br />swing joints, fittings, clamps, bands, and pumps shall have corrosion protection. <br />19. A "Certification of Installation / Modification" form for each tank shall be submitted by the <br />owner or the owners' agent [pursuant to California Code of Regulation (CCR) 2635(d)] <br />before this install will be considered complete by EHD. <br />20. An accurate and drawn to scale as-built plan must be submitted to the EHD prior to <br />scheduling a final inspection. <br />21. No gasoline or other hazardous material or waste can be released (intentionally or <br />unintentionally) to the sewer/septic, storm water system, or any other waters of the state. <br />If a release occurs it will be considered illegal hazardous waste disposal and will be <br />subject to enforcement. <br />22. Submit the Best Management Practices (BMP) to EHD for managing fuel spills and <br />wash-down water at this site prior to the final inspection. <br />23. Submit to CERS correct and complete Monitoring and Response Plans prior to <br />scheduling a final inspection. <br />24. Submit to CERS a Certification of Financial Responsibility for Underground Storage <br />Tanks containing petroleum form with the appropriate mechanism prior to scheduling a <br />final inspection, and all other applicable CERS submittal components. <br />25. Passing Enhanced Leak Detection (ELD) results must be received prior to receiving fuel. <br />26. This permit expires at the end of the next calendar year— December 31, 2017.
