The 87, 91, and diesel pipe monitoring is performed <br /> using the following methods: Leak alarm triggers automatic pump shutdown and Failure/disconnect triggers pump <br /> shutdown. These have been left blank. These should be marked as yes. The monitoring plan must be uploaded to tf- <br /> California Environmental Reporting System(CERS). Immediately log into CERS, make the necessary changes, anc <br /> submit for review by the EHD. <br /> This is a minor violation. <br /> 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br /> Maintenance and monitoring records for the last three years are incomplete. Sensors L1 through L10 alarmed on <br /> 12-9-15. The facility and the designated operator did not document these alarms. These records shall be maintained <br /> on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling, (2) <br /> monitoring equipment calibration and maintenance records; (3) results of any visual observations;(4) results of sample <br /> analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water <br /> elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain <br /> all maintenance and monitoring records for the last three years on site and submit copies to the EHD. <br /> This is a minor violation. <br /> 204 CCR 2712() Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible <br /> on site at all times. Immediately locate and retain a copy on site. <br /> This is a minor violation. <br /> 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to document all the alarms from the attached alarm history on the 1-21-16 designated <br /> operator monthly inspection report and failed to check that they were responded to appropriately. The missing alarms <br /> include: Sensors L1 through L10 that alarmed on 12-9-15. During the monthly inspection, the designated operator <br /> shall review the alarm history for the previous month, check that each alarm was documented and responded to <br /> appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the <br /> monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the <br /> required information on the reports. <br /> This is a minor violation. <br /> Page 4 of 6 <br />
