San Joaquin County <br />Environmental Health Department <br />1868 East Hazelton Avenue, Stockton, California 95205-6232 <br />Telephone: (209) 468-3420 Fax: (209) 468-3433 Web: www.sigov.org/ehd <br />Underground Storage Tank Program Inspection Report <br />Facility Name: <br />FAST N ESY <br />Facility Address: <br />1399 E YOSEMITE AVE, MANTECA <br />Date: <br />June 08, 2016 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />205 <br />CCR 2712 Failure to comply with UST operating permit conditions. <br />1) One of the turbine relays was replaced on 2-12-15 and a permit was not obtained from the EHD (per the Donlee <br />Pump Company (Donlee Pump) work order, no indication as to which product relay was replaced was provided on <br />that work order). This is a historical violation, first noted on the 6-11-2015 Environmental Health Department (EHD) <br />inspection report. A return to compliance certification dated 11-17-2015, indicated that this facility is working with <br />Donlee Pump to resolve the issue, however, to date no permit application for the above work has been received by <br />EHD. Emergency repairs relating to UST system monitoring and release detection may be performed by a properly <br />licensed, trained and certified contractor pursuant to EHD's policy and a permit obtained within one business day <br />from the EHD. All other repairs must have a permit submitted to and approved by EHD prior to beginning of work. <br />Immediately obtain a permit from the EHD for the replaced component. A "Failure to Obtain a Permit" penalty applies. <br />2) According to the onsite maintenance records the following repairs were made by Donlee Pump without first <br />obtaining a permit from EHD for these repairs: <br />a) L8 sensor in the 87-octane annular appears to have been replaced according to the maintenance record dated <br />6-29-2015, which indicated that this sensor was malfunctioning and a part was ordered and the sensor must be <br />replaced. Several "sensor out" and "fuel alarms" have occurred on 7-2-2016, however no maintenance record, which <br />indicated a sensor replacement was available for review. L8 sensor passed the testing during today's inspection, <br />without any noted issues. <br />b) A shear valve was replaced UDC'/, (91-octane product line), per the 11-19-2015 Donlee Pump maintenance <br />record <br />c) L10 annular sensor was replaced, per the 12-19-2015 Donlee Pump maintenance record <br />d) 87-octane line leak detector failed and was replaced, per the per the 01-04-2016 Donlee Pump maintenance <br />record. Also a seal was replaced on the 91-octane line leak detector and was replaced, also noted on the 01-04-2016 <br />Donlee Pump maintenance record <br />Emergency repairs relating to UST system monitoring and release detection may be performed by a properly <br />licensed, trained and certified contractor pursuant to EHD's policy and a permit obtained within one business day <br />from the EHD.
