The designated operator <br />inspection reports for July 2015 and December 2015 indicated N/A on the first page of the inspection report for <br />alarms which occurred in their respective months. During the monthly inspection, the designated operator shall <br />review the alarm history for the previous month, check that each alarm was documented and responded to <br />appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the <br />monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of <br />the required information on the reports. <br />2) The designated operator failed to check and note the correct test dates for the monitoring system certification on <br />the August 24, 2015, September 11, 2015, November 17, 2015, December 9, 2015 and May 10, 2016 designated <br />operator monthly inspection report. The monitoring system certification date was noted as June 10, 2015 instead of <br />June 11, 2016. During the monthly inspection, the designated operator shall check that all required testing and <br />maintenance for the UST system have been completed and document the dates they were done. Ensure that <br />designated operators performing monthly inspections at this facility are including all of the required information on the <br />reports. <br />This is a repeat violation, Class II. <br />306 <br />CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br />The 87 STP sump sensor was raised approximately 8 -inches off the lowest point of the sump (located up the sump <br />saddle) and diesel SPT sensor was located approximately 4 -inches off the lowest point of the sump (located up the <br />sump saddle) and not located to detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to <br />be able to detect a leak at the earliest possible opportunity. The sensor was relocated to the lowest point of the sump <br />by the service technician. Ensure that all monitoring equipment is maintained to detect a leak at the earliest <br />opportunity. <br />This is a repeat violation, Class ll. <br />317 <br />CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. <br />The 87 -octane line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of <br />detecting a 3 -gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, and certified contractor <br />repair or replace the failed leak detector (LG 113-x listed, if applicable) and obtain a permit within one business day <br />from the EHD. If the failed leak detector can't be replaced immediately, there is a possibility that the 87 UST system <br />may be red tagged to prevent fuel inputs. <br />This is a repeat violation, Class II. <br />Page 6 of 7 <br />
