This is a revised version of the original underground storage tank <br />(UST) inspection report issued on June 5, 2014. This report supersedes all previous versions. <br />Class II Violations: 105, 106, 310 <br />105. UST Facility Operating Permit Application and UST Tank Information are not uploaded in the <br />California Environmental Reporting System (CERS). Immediately log into CERS, upload the required <br />information, and submit for review by the EHD. Any change of information must be updated in CERS <br />within 30 days of the changes. Complete a separate tank form for each tank. <br />106. The monitoring plan is not uploaded to CERS. Immediately log into CERS, upload the required <br />information, and submit for review by the EHD. Complete a separate record for each tank. Maintain an <br />approved copy on site. <br />310. Liquid was observed in the UDC 1/2 (less than '/z cup) and in diesel STP sump (1-2 cups). If <br />water could enter into the secondary containment by precipitation or infiltration, it must be removed and <br />disposed of properly. The technician onsite removed the observed liquid and retained for reuse. Ensure <br />that the all sumps are maintained free of liquid. CORRECTED. <br />Minor -Notice to Comply: 204, 303 <br />204. The designated operator failed to document all the alarms from the attached alarm history on the <br />3-6-2014 designated operator monthly inspection report and failed to check that they were responded to <br />appropriately. The L7 alarm, which had occurred on 2-28-2014, was documented as if it had occurred <br />on 2-20-2014. During the monthly inspection, the designated operator shall review the alarm history for <br />the previous month, check that each alarm was documented and responded to appropriately, and attach <br />a copy of the alarm history with documentation taken in response to any alarms to the monthly report. <br />Ensure that designated operators performing monthly inspections at this facility are including all of the <br />required information on the reports. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />Inspector.
