The diesel STP sump sensor was located approximately 5 -inches off the lowest point of the sump <br />and not located to detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to <br />be able to detect a leak at the earliest possible opportunity. The technician onsite repositioned the <br />sensor to the lowest point to detect a leak at the earliest possible opportunity. CORRECTED. <br />Note: Facility operators are required to upload the following UST program documents into the <br />California Environmental Reporting System (CERS): UST Facility Operating Permit Application and <br />UST Tank Information, UST Tank Information/Monitoring Plan, UST Certification of Financial <br />Responsibility, UST Response Plan, UST Owner/Operator: Written Agreement (if applicable), UST <br />Letter from Chief Financial Officer, and Owner Statement of Designated UST Operator Compliance. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of <br />how each violation was corrected, including supporting documentation, by July 19, 2014. <br />Provided: <br />-Return to Compliance Certification form <br />-Schedule of Free Classes <br />Discussed: <br />-CERS requirements and the necessity for the EHD office appointment scheduling <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector <br />�(Vla►n / <br />Received By: <br />via vvt <br />Title: <br />EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
